Nomination letters are to be submitted on Fire Department or Volunteer Ambulance Corps letterhead and must be signed by either the Chief of the Department or the EMS Chief/Captain in charge of the incident for VACs in order to be considered.
No exceptions.
Additional support, such as newspaper clippings, personal accounts, etc., is encouraged!
*Send nominations for incidents occurring between Jan. 1, 2025 and Dec. 31, 2025 with this completed application and requirements as noted above no later than March 1, 2026 for consideration.
